Open Wounds is a combat mechanic that appears as a Chance on Hit mod on several, or items. The game will roll for the chance for the mechanic to occur on every eligible melee or ranged attack. If the effect occurs, it causes the monster to bleed profusely, causing them to lose health. The effect lasts for 8 seconds.Health Lost.

The amount of health the monster loses is scaled with the player's level. It scales fairly linearly, doing about 800 damage over 8 seconds at level 50, and about 2135 damage over 8 seconds at level 90. When triggered by a ranged attack, the bleed damage is divided by 2. When targeted on a player, the bleed damage is divided by 4. (So 1/8 the damage for ranged attacks on players). When targeted on Uniques or Bosses, the bleed damage is divided by 2 (Causes 1/4 damaged for ranged on bosses).
Item modifiers for Open Wounds generally stack, unless a character is wielding two weapons, the chances do not cross between weapons.Health RegenerationWhile under the effects of Open wounds, a Monster's natural health regeneration is set to a negative value. This means that monsters cannot regenerate health while affected by Open Wounds. This makes it a valuable stat for fighting monsters with high health regeneration.Diablo II Item Mods.
You need to get 9 keys, 3 each of Terror, Hate, and Destruction from The Countess, The Summoner, and Nihlathak in Hell. These will allow you to open 3 red portals, where you will fight Uber bosses to get three more special items.

The Keys of Hate, Terror, and Destruction arerequired, and can only be found from certain boss monsters, and onlyon Hell difficulty. Get the Key of Terror from The Countess, the keyof Hate from The Summoner, and the Key of Destruction from Nihlathak.It's a good idea to have 3 of each key before beginning the secondstage of the Pandemonium Event, so you can do all three of the specialareas in the same game. (The same area will not repeat in the samegame, so you'll have all 3 ingredients for Uber Tristram.) The keyslook identical, and can only be discerned between by hovering on themto see their name.Key of Terror These are dropped by The Countess, who is located on thefifth level of the Forgotten Tower in the Black Marsh, Act One. She iscommonly run for her rune drops, and will drop a Key of Terror about8% of the time she dies on Hell difficulty.Key of Hate The Summoner drops this one. He's found at the end of oneof the four points of the Arcane Sanctuary, in Act Two.

He's found at one end of the four-pointed Halls ofVaught, the lowest of the three Halls levels reached through Anya'sportal in Harrogath, in Act Five.Opening the Red PortalsOnce you have the keys, stand in Harrogath, on Hell difficulty, placeone of each in your Horadric Cube and convert them. A red portal willopen to one of three locations; the Matron's Den, the Forgotten Sands,or the Furnace of Pain.
Which portal opens first is random, but if youdo all three in the same game you will not get the same one twice. Forthis reason it's recommended that you have 9 keys to begin with, 3 ofeach type, so you can do them all in the same game.
It's alsorecommended that you stand in different places so you'll spread outthe three portals, so they don't overlap and make selecting thecorrect one 'uber difficult'.
